Program Locations

Abbreviation Key for Models of Care

Social:

Daytime services for older adults and people with disabilities where they participate in planned stimulating activities and interaction with other participants. Social programs include ADP and some are called Community Respite Programs.

Medical:

Daytime care for older adults and people with disabilities who require health services prescribed by their doctor. Social interaction with other participants and goal-oriented activities are included in this program. Called Adult Day Health Care.

Community Respite Care:

Daytime care that provides activities and socialization with some supervision. Allows the caregiver time away from caregiving responsibilities. May not be a licensed program.

Definitions

ADP:

Adult Day Program / Social Day Program. A program that offers daytime care (between four and eight hours) for frail seniors in a group setting.

ADHC:

Adult Day Health Care. A licensed program that provides daytime care including social and health services in a group setting, specific to the individual’s need and based on a care plan.

ADCRC:

Alzheimer Day Care Resource Center. A licensed community-based program that provides specialized daytime services for persons with mid to late stages of Alzheimer’s or other dementias. The program also provides support and education to the family members and caregivers and, as requested, to the community at large.
